How to choose a physician recruitment firm

The right physician recruitment firm depends on what you’re hiring for and how your organization works. A 400-bed health system, a three-physician private practice and a concierge medicine group need very different things from a search partner. These ten questions will show you quickly whether a firm fits.

10 questions to ask before you sign

  1. Is permanent search your core business? Some firms mainly staff temporary coverage. If you need a permanent hire, look for a firm whose main business is permanent search.
  2. Who will actually run my search? At some firms, the person who sells the search hands it to a junior recruiter. Ask for the name of the person who will call candidates.
  3. How many searches does that person carry? In-house physician recruiters carried a median of 36.5 searches each in 2025 (AAPPR). Ask how much attention your role will get.
  4. How do you find candidates who aren’t job-hunting? The strongest candidates are rarely on job boards. Ask what share of their placements came from the passive market and how they reach those physicians.
  5. Have you recruited for organizations like mine? A private practice partner-track hire, a DPC physician and a hospital-employed specialist are different searches. Ask for examples and references in your setting.
  6. How do you learn our organization before sourcing? Look for a firm that wants to understand ownership, culture, comp, call and community — not one that asks for a job description and starts sending CVs.
  7. How will you screen candidates before we see them? You should only see physicians who are interested, qualified and realistic about the role.
  8. How are your fees structured, and when are they due? Ask what you pay if the search doesn’t produce a hire, and how much is due before you see a candidate. See what physician recruitment costs.
  9. What happens if the physician leaves early? Ask about guarantee or replacement terms, in writing.
  10. Are you a member of a professional association? Membership in the National Association of Physician Recruiters (NAPR) signals a commitment to professional and ethical standards.

Frequently asked questions

What is the best physician recruitment firm?

The best firm is the one built for your kind of hire. Large national firms suit high-volume needs. Boutique, partner-led firms like Capstone Recruiting Advisors suit organizations that want a permanent hire and a close, high-touch search.

What is a boutique physician recruitment firm?

A smaller firm where senior recruiters or partners run each search personally, take fewer searches at a time, and tailor the process to each client.

Should a private practice use the same recruiter as a hospital?

Not necessarily. Practices need a recruiter who can sell partnership, ownership and autonomy, and who understands a small group’s budget and culture.
